How do I obtain Laser Vision Correction?
Call any of the offices on the Dakota Eye Care provider list and explain that you would like to make an appointment for a preliminary work-up for laser vision correction. During this examination, the doctor will determine whether or not you are a good candidate for the procedure. If you are, your primary eye doctor will schedule an appointment for you with the Dakota Eye Care refractive surgeon of your choice. Follow-up appointments after the surgery will be scheduled back with your primary eye doctor.
 
Are implants or other alternatives to LASIK covered under your Laser Vision Correction plan?
Yes, to give you the most options in determining which corrective procedure is right for you, all of the following are covered:
    Conductive Keratoplasty
    LASEK
    LASIK
    Phakic Implants
    Photokeratectomy
    Refractive Lens Exchange

Which doctors accept this card?
This card is good at any of the doctors listed in our network.
 
Are eye exams covered?
Yes, regular eye exams for glasses, contact lenses, and laser vision correction are covered, but medical, surgical, and other laser procedures are not. Neither are disposable or planned replacement contact lenses, industrial safety glasses, and non-prescription sunglasses.
 
Are bifocal lenses and frames covered?
Yes, bifocals, trifocals, progressive add (no-line bifocal and trifocal) lenses, photochromatic lenses, prescription sunglasses, ultra-thin lenses, standard contact lenses, and frames are all covered.
 
Do I have to submit a claim form to receive these benefits?
No. Simply present your Dakota Eye Care discount card to one of the participating doctors' offices and your normal charges will be reduced by 20% (some restrictions apply, see above) when you take care of the bill. There is no additional paperwork for you to complete.
 
How often can this benefit be used?
There is no limit to the number of times you and your covered dependents can use the Dakota Eye Care discount plan. You may obtain eye exams, multiple pairs of prescription sunglasses or eyeglasses, contact lens exams, and standard contact lenses (sorry, disposable and planned replacement contact lenses are not covered) as often as you wish before the expiration date of your card.
 
What if I lose my card?
Be sure to bring your card with you because you must present it to your doctor to receive benefits. If necessary, replacement cards can be purchased for a $3 handling charge. Contact Us if you need to order a replacement.
 
Is my whole family covered, or just one person?
You may purchase a family card or an individual card. Current prices for each are listed on our Order Forms. Laser Vision Correction cards are only available individually.
 
Are the discount cards renewable?
Cards may be renewed annually. All card members will be sent annual renewal reminders.
 
Who qualifies as a dependent for a family plan?
A dependent is a spouse or an eligible child who is [unmarried and not yet 19] OR [unmarried, not yet 24, and a full-time student].
